15% used to be the standard for sit-down service; 18–20% is now more common for good service. 15% remains acceptable for poor service or buffets.
Sit-down service: 18–20% is normal, 20–25% for great service. Bartenders: $1–$2 per drink or 20%.
Common practice is to tip on the pre-tax subtotal, but tipping on the total is fine too — the difference is small.
